We accept most insurances, both private and public, as well as DHS contracts. If you are a person without insurance, we also accept sliding fee scales based on individual income.

Sliding Fee Scale/ Escala móvil de tarifas:
Services of Hope offers a Sliding Fee Scale program which allows patients to pay a reduced fee for most SOH services. This program is based on your family size and income. It is not an insurance plan. To view and print the Sliding Fee Scale application, please click the link below. We need an identification and proof of your family income information as shown on the application. At the time of your visit, one of our office support specialists will be happy to help you complete the application and answer any questions you may have. No one will be denied due to inability to pay.
